§ 139.4. Registration.
(a) An owner intending to operate or use an amusement ride or an amusement attraction in this Commonwealth during a calendar year shall register with the Department prior to operation.
(b) An amusement ride or attraction which has undergone major modification shall be registered and inspected by a qualified inspector before its operation for use by the public.
(c) Registration includes the following:
(1) Owner’s name, address and telephone number.
(2) Type of activity, park, carnival or fair.
(3) List of each ride or attraction by ride name, manufacturer’s name and serial number.
(4) Name of owner’s insurance company.
