§ 6500.115. Content of the IPP.
Each IPP shall include:
(1) Goals for the individual in specific areas of growth and development.
(2) An evaluation of the individuals skill level for each goal.
(3) Monthly documentation of the individuals progress on each goal.
(4) A plan for the individual to participate in community and family life.
(5) Specific activities and services that meet the needs of the individual.
(6) A program and schedule for allowing the individual to be without direct supervision for specific periods of time, aimed at achieving a greater level of independence, if the individuals assessment states the individual may be without direct supervision.
(7) A planned program to address the social, emotional and environmental needs of the individual, if a medication is prescribed to treat maladaptive behavior.
