§ 20.55. Preparation of certificate of compliance.

 (a)  The certificate of compliance is issued to the legal entity.

 (b)  The certificate of compliance lists the name of the legal entity, the name and address of the facility or agency, satellite sites, if applicable, type of service provided, maximum capacity, title and chapter of applicable licensure or approval regulations, the date the regulations were adopted, certificate number, effective and expiration dates of the certificate, and restrictions, if applicable.

 (c)  A provisional certificate of compliance also contains the word ‘‘Provisional’’ and indicates whether it is the first, second, third or fourth provisional certificate issued.
